Deaf Mentor Family Program Application

Grant funding for this program is limited. Most often, mentor services are paid for with state grant funding through the Minnesota Department of Human Services Deaf and Hard of Hearing Services Division (DHHS). If your family is interested in receiving mentoring services for the purpose of learning sign language to communicate with your child please complete the form below. By completing this form you will be helping the Deaf Mentor Family Coordinator assess the need for Deaf Mentor services.

The Deaf Mentor Family Program is primarily an "in home" service. Some of the lessons could be taught via "video mentoring". In order to consider this as a service option, do you have a computer?
